Medical Plan Documents

The Health Benefits Trust offers five different medical plans along with a dental and vision plan. If you’re not certain which plan you’re covered under, check with your credit union or review your medical ID. Click on the “Summary” link for the plan Summaries of Benefits and Coverage and the “Plan Document” link for the Plan Document. Both Summaries of Benefits and Coverage and Plan documents for 2025 are in the process of being updated, in the meantime, please consult the 2024 Plan Documents.
